It would be beneficial to you and your family if you composed a list of contacts in case one of your family members needs information. This is not an easy topic to address but we are getting older and may need some help from those trusted by us.
On several occasions we at CRTC have received questions regarding one of our retirees from family members. We can only provide them the current information that you supplied on the annual membership form. So, it may be helpful to compose a list of people to be notified if you are not able to do that yourself:
• CRTC: Notify CRTC if there is a change in your membership information or status.
• Commack Public Schools: Contact the Commack Business Office regarding your Health Insurance Premiums that are paid to the district.
• NYSUT: Notify NYSUT if you have any Member Benefits that you pay for or are deducted from your NYSTRS pension check. Put your NYSUT member number on the list of member Benefits Policies that you may have.
• NYSTRS: Notify the New York State Teachers Retirement System with questions or termination of payments. Put your retirement number or save the copy of your annual Retired Member Profile that is mailed out in September each year.
• Friends: If you have friends that you want notified should that time arise, make a list so your family can let them know.
